Your brain registers a stubbed toe, a sunburn, and a headache, yet the brain tissue itself contains no nociceptors — the specialized nerve endings that detect pain. This anatomical quirk makes one of medicine’s most remarkable procedures possible: awake brain surgery. During an awake craniotomy, the scalp, skull, and the pain-sensitive dura are numbed with local anesthetic, but the patient stays conscious while surgeons work directly on exposed brain.

Why bother keeping someone awake? Mapping.
