The Brain Itself Cannot Feel Pain
The organ that processes every ache in your body has no pain sensors of its own, which is why surgeons can operate on it while you are wide awake.
Your brain registers a stubbed toe, a sunburn, and a headache, yet the brain tissue itself contains no nociceptors — the specialized nerve endings that detect pain. This anatomical quirk makes one of medicine’s most remarkable procedures possible: awake brain surgery. During an awake craniotomy, the scalp, skull, and the pain-sensitive dura are numbed with local anesthetic, but the patient stays conscious while surgeons work directly on exposed brain.

Why bother keeping someone awake? Mapping. By gently stimulating the cortex and asking the patient to talk, count, or move, the surgical team identifies which areas control speech and movement, steering around them while removing a tumor. The headaches you feel originate not in the brain but in surrounding blood vessels, membranes, and muscles — the brain merely interprets those signals for the rest of the body.
